Bank of America. Also, you can have a family photo on the card as a design. The ID photo appears on the left top corner.

USAA will let you put your photo on the card. Shortly after I was approved for a Mastercard, I received information in the mail on how to obtain a photo ID credit card. I couldn’t think of where to get a (per the instructions) 2″ by 2″ photo (I don’t own a camera). I stopped by Bank of America, and, although I’m not a customer and offered to pay for the photo, a bank employee very kindly didn’t charge me for taking a picture of me. The photo turned out to be about an 1/8″ too short on the left and about 1/8″ too short on the right. Consequenty, when I received my photo ID credit card about two weeks later, the picture appeared to have turned out tinier than maybe it otherwise would have if I had gone elsewhere (perhaps Kinkos) and obtained a photo that was exactly 2″x2″. Overall, though, it still turned out pretty well.

Besides the photo security credit card that creditnewbie mentioned, Bank of America has something called “photo expressions”:

Capital One has the “image card” (unfortunately at this time this option is not available to Mastercard cardholders):

First National Bank of Omaha has the “One of a Kind Visa” card:

I found this with a google search:
America First Credit Union

While I was at a store, the person in front of me paid with a Chase card that had his photo on it. I don’t know if it was a Chase credit card or Chase atm card.

For $9.95 you can purchase a program known as Passport Photo which will take any digital photo and allow you to print it in the correct passport size for US, Canada, and a few other countries. I used it for my passport photo.. Came out just fine.
