Have you tried the most basic tactic - have you called customer service and explained the situation? As I recall from having dealt with them a few years back, they're very good and will do their best to help you resolve the situation.

From the adverse action, even though you say you don't have an account with AT&T, you should be able to call the 800 numbers for the CRA and go through the automated system for adverse action to aquire a free copy of your credit report, if you are still within 60 days of the action.
You need that credit report first before Equifax customer service can be accessed. Their number is 800-685-1111, otherwise you will need to purchase a copy of your credit report.
Another thought, is to treat this as ID Theft, until you can determine what the actual problem is, 800-525-6285 this will activate fraud alert for 3 month period, forward you a copy of your credit report, and automatically opt you out of receiving junk mail.
Calling the Creditor directly is recommended to, to discuss the problem as to why they are trying to collect on a debt with you which you don't have.
If you find it is a problem with ID Theft, you should check out:
www.ftc.gov for further instructions as to handle the problem properly. And one final thought, you might even have a mixed file, with another person who actually owes this debt. Mixed file can happen if you have a common name, or even from clerical error on a creditors part. If that is the case, then forwarding a copy of your drivers license you clear the problem with the reporting agency / with request to unmix the file.
Just a few possibilities here,
